Please download the data form through the link above. ONLY THE OFFICIAL DATA FORM WILL BE ACCEPTED. If you cannot access scribd, click this (Fileden).

SUBJECT of EMAIL: Yearbook: batchnumber[space]surname[space]initials
Example: Yearbook: 201 Senga JRL

*Please don't forget to follow the format (especially putting "Yearbook:" because it acts as a filter. Submissions not following the format will automatically be transferred to the trash bin.

FILE NAME of WRITE-UP: batchnumber[space]surname+initials[space]WriteUp
Example: 201 SengaJRL WriteUp

FILE NAME of DATA FORM: batchnumber[space]surname+initials[space]DataForm
Example: 201 SengaJRL DataForm

For samples of yearbook write-ups, click the ff:

• 1st person write-up (if you are writing for yourself)
• 3rd person write-up (if other people are writing for you)

If you are not able to submit your write-up by the deadline, a template will be used by the editors to construct a generic write-up for you, so please submit on time.
